Principal Benefits and Covered Services

  • Deductibles: $50 per person, $150 per family per calendar year.
  • Benefit Maximums: $2,500 per person per calendar year.
  • Diagnostic and Preventive Benefits: 100% of Delta Dental dentist’s fee (the deductible does not apply to these services).
  • Basic Benefits: 80% of Delta Dental dentist’s fee.
  • Prosthodontic Benefits: 50% of Delta Dental dentist’s fee (bridges, dentures).
  • Orthodontic Benefits: 80% of Delta Dental dentist’s fee (subject to a $2,500 lifetime maximum per person).

How to Receive Care

Under your Delta Dental Premier® plan, you may visit any licensed dentist or dental specialist of your choice, change dentists at any time and receive dental care anywhere in the world.

The following are some of the advantages of choosing a Delta Dental Premier dentist:

  • Premier dentists won’t bill you for any amount over their Premier fees. You are responsible for any applicable deductibles, coinsurance, amounts over plan maximums and charges for non-covered services.
  • Claim forms are completed and submitted for you at no charge.
  • You can visit any licensed dentist, but your out-of-pocket costs may be higher if you choose a non-Delta Dental dentist.

It’s easy to find a Delta Dental dentist. Simply:

  • Go to deltadentalins.com and look for the Find a Dentist tool on the right side.
  • Enter a location and select your plan (Delta Dental Premier).
  • Click Search. (You can also filter your search results by categories, such as specialty, language, gender, extended office hours and accessibility.)
  • To see if your current dentist is a Delta Dental dentist, search by the name of the dentist and enter “All of the above” for the network.

If you choose a non-Delta Dental dentist, you may have to complete and submit your own claim forms or pay a service fee. Delta Dental has no fee agreement with non-Delta Dental dentists, so you are responsible for any difference between the non-Delta Dental dentist’s charges and Delta Dental’s payment. In addition, you may have to pay the entire bill at the time of treatment and wait for reimbursement.

About your Delta Dental ID Card and Enrollee ID Number

You’ll receive two ID cards after you are enrolled in the plan. Your ID card will have your group name and number, your name and your Social Security Number or Enrollee ID Number. The card is for information purposes only and is not required to obtain services. You may show your ID card or provide your Delta Dental group number (00805) and your Social Security Number or Enrollee ID Number.

Go Paperless

You can check your eligibility and benefits details, access your claim statements and get an email alert every time you have a new claim. Simply visit deltadentalins.com and, if you haven’t already done so, register for Online Services. Already have an account? Log in with your username and password. Then:

  1. Click the My Profile tab.
  2. Go to Document Preference (after the username and password questions).
  3. Select Online with Email Alerts. Click Save.
